  • Right to Refuse Service: We reserve the right to deny or discontinue treatment to any individual at any time if our staff determines, in their professional judgment, that a service or therapy is contraindicated, unsafe, or medically inappropriate for your health profile.

3. Booking, Cancellation, and Refund Policies

By scheduling an appointment with Oak & Sage Wellness Center, you agree to our scheduling and financial terms:

  • Cancellation Window: We require at least 24 hours’ notice for any appointment cancellations or rescheduling requests.
  • Late Cancellation & No-Show Fees: Cancellations made with less than 24 hours’ notice, as well as “no-show” appointments, will be subject to a cancellation fee up to the full cost of the scheduled service.
  • Refunds: Services, custom peptide orders, and completed treatments are non-refundable. If you experience an unexpected adverse reaction or issue, please contact our clinic management immediately.

 

4. Limitation of Liability

To the maximum extent permitted by applicable law in the United States, Oak & Sage Wellness Center, its owners, clinicians, employees, and affiliates shall not be liable for any direct, indirect, incidental, consequential, or punitive damages resulting from:
